Fill Out a Referral Form Online

For Veterinarians

As an extension of the primary care veterinary community, we are proud to assist referring veterinarian’s in providing optimum medical care for their clients and patients. If you are a licensed veternarian and would the option to refer your patients to us, contact us to set up you online account.

Our web-based referral system provides multiple, clear referral options to referring practices while adhering to industry best practices and regulatory requirements.

These online forms will provide quality assurance to both referring practices and our medical teams – ensuring all pertinent data specific to an individual patient’s case has been shared efficiently and in a legible, printable format.

Our referral form details the necessary documentation we require about your client and patient. After the referral is received and reviewed, we will contact your client via the phone number recorded on the referral to schedule an appointment.

If you need to speak to a service team member regarding a referral, please call us. Please note, emergency referrals should always be followed up with a phone call to our main line at 709-221-7838.

Radiograph Submission

If emailing images applicable to a referral, please indicate on the referral that images will be transmitted via email. Images must be emailed to referrals@vscnl.ca

Images should be in DICOM format to ensure quality and visual enhancement is not compromised. Other formats (PDF, JPEG, Bitmap, TIFF, etc.) may impact adequate interpretation.

Each images should include patient name, veterinary practice name, and date of radiograph. If multiple images are included, each must be uniquely identified.

In the body of the email, please include:

  • Clinic name and phone number
  • Patient name
  • Client first and last name
  • VSCNL service to which you are referring (i.e., Diagnostic Imaging, Surgery, Emergency)
  • Date and time the images were acquired
  • Email images to referrals@vscnl.ca

If you are unable to send DICOM images, we recommend contacting your IT support for assistance. Almost all popular workstations have this functionality and once the setup screen is accessed, sending DICOM images is usually relatively straight forward. The workstation must be able to access the internet. Sometimes firewall modifications are necessary.

Imaging studies being submitted solely for radiographic interpretation must be submitted with a Diagnostic Imaging – Interpretation referral.

Patient Discharge

Our intent is to always provide you with a case summary within 24 hours of discharge. If you have not received a patient records, please contact our main line at 709-221-2838.
