Before you start

Our referral form details the necessary documentation we require about your client and patient. After the referral is received and reviewed, we will contact your client via the phone number recorded on the referral to schedule an appointment.

Please note, emergency referrals should always be followed up with a phone call to our mainline at 709-221-7838.

If you would like to transfer a patient to our ICU for overnight care, please call us directly at 709-221-7838.
